What to Expect

At your first visit, your physician will take the time to learn about you and your specific needs. They may perform a physical exam, or recommend additional tests in order to determine a diagnosis. They will then go over treatment options with you.

Please arrive 15 minutes prior to your appointment. You may also print and fill out a new patient form before your visit:

Insurance Information

We accept Medicare, Medicaid, and almost all commercial insurance as payment. Our billing staff is happy to help patients who may require a payment plan. For questions related to insurance or billing, reach out to us at (800) 609-4036.

Copayments and outstanding balances are expected to be paid at the time of service. We appreciate a 24 hour cancellation notice, otherwise a fee may apply.

Please note that health insurance does not cover auto-accident related treatment.
